The “Elevator Consult”

In this activity, you will practice giving a synopsis of your patient to your preceptor. In practice, you may often give this type of report if you are sending a patient for a consultation and your phone the specialist to discuss the patient. This report should be concise and clear. The receiver should, within one minute (slightly less for simple cases, slightly more for complex cases) have a picture of the patient in his/her head. You will report on ONLY items pertaining to the acute problem in this case. Do not include extraneous material or material not directly impacting the decision-making regarding this problem. Remember, this is a FOCUSED visit and assessment to evaluate a focused concern. The history and physical exam applies techniques relevant to the specific complaint for the patient at that visit. Your report should be similarly focused, providing only information that relates specifically to the presenting problem.

Background: During America’s Age of Imperialism in the 1890s, the United States aggressively pursued overseas colonies, holding on to those colonies even in the face of indigenous resistance. Unlike its handling of continental territories, the United States offered the new colonies no pathway toward equal statehood and citizenship.

American Imperialism combined the expansionist ideology that propelled Americans from the Atlantic to the Pacific Oceans with a desire to become a world power as well as the need for new markets and raw materials to feed the growing industrial base. Inspired by Alfred Thayer Mahan’s concept of sea power, Americans began to look outside their borders for the means to grow their global political influence. Fueled by the technological innovations and cheap labor of the Industrial Revolution, American industry looked abroad for new markets and access to natural resources. An early and vociferous proponent of American Imperialism, Theodore Roosevelt aggressively and effectively promoted the cause through initiatives like the construction of the Panama Canal and the demonstration of American military power embodied by the Great White Fleet. With the articulation of the Roosevelt Corollary to the Monroe Doctrine, Roosevelt claimed the U.S. right to keep European powers out of Latin America through the use of military force.

As you read, look at how you (taking on the persona of the theorist) developed the theory. In the first stage, theorizing occurs. This is where you, as the theorist, identify the concepts of what nursing is and is not. Perhaps you questioned what concepts were guiding those in nursing practice and then started to question your role. You started asking yourself, “Where am I in nursing, and where the profession is going? Is there some overarching concept that guides the professional in his or her practice?” This is where you recognize that a theory is needed. In the second stage, syntax is developed. This is where you will define the terms. Look for changing definitions of terms. Consider, for example, Jean Watson. She starts by defining the word “caring.” Within the last ten years she has refined her terminology changing the term caring to caritas. This demonstrates a growth and maturation from decades of research she and other scholars did to produce the theory of caring. The third stage is theory testing. Defined as the phenomena behind the theory that are exposed through research. The definitions of terms are refined. The theorists and other researchers consider whether this theory helps answer questions that arise in practice. This is where your theory is used in by a widening group of researchers. For example, graduate nursing students request the tool you developed while testing your theory. The fourth and last stage is evaluation. This is where philosophical debate occurs as the concepts are applied through evidenced based practice in the act of providing nursing care. Background: Environmental economists are concerned with identifying specific problems to be rectified, but there can be many approaches to solving the same environmental problem. If a state is trying to impose a transition to clean energy, for example, they have several options they can take. Task: Assuming you are the President of a State University such as Rutgers (that has a student population of around 30,000 and a staff population of 8,000 with 3 main satellite campuses), and you would like to make the University greener. An initial environmental audit identified several problems that required urgent attention. Problems of note included a high and increasing energy bill each year (linked to poor energy conservation practices and the use of inefficient and outdated lab equipment and lighting infrastructure), high waste generation particularly grey water and PET containers and a fairly high annual water utility bill (a large portion of which is due to leakages and poor conservation practices by the University community). Suggest three (3) strategies you would recommend as President to achieve your goal(s) of making the University greener. For each strategy, provide a brief description of how that particular action would be rolled out, resources necessary for it to work, possible challenges, and possible indicators of success.
