Key Functional Areas of My Nursing Informatics

I currently work in a Wellness office that creates macros in a cloud based web software called practice fusion. This software requires users to be proficient in at least level 3 informatic competencies as described by Staggers (2012). Important skill sets that are required at this level is the ability to create macros, manage software, ability to integrate other web based platforms, analyses the health and safety of the software to name a few important parts of my job.

Functional Areas That Need Improvement

In EHR the biggest draw, back in my opinion, is speed and efficiency. In the current system entering and collecting information takes long periods of time. Currently in development is the integration of tablet based questioners that patients could fill out upon arrival at the office. The hope is to have this web based which would allow the patient to enter the information from home before coming to the visit. This would allow for more prep time for the physicians.

Summary

One quote from our reading that stood out to me was, “One challenge that has been identified in the literature and continues to plague health care is the vast differences in computer literacy and information management skills that healthcare workers possess” (McGonigle, 2014). Competencies must always be a major goal of today’s healthcare provider.

These competencies need to be improved by always questioning one’s method and evaluating the time it takes from beginning to end of a patient visit. This information is essential in all offices because it can be used to determine profitability potentials. This can only be done with trial and error and extra time spent in the office up front to try and save time later. The IOM also reminds nurse’s that effectiveness alone is not enough. We need to promote safety and effectiveness in our practices (Murphy, 2010).

Key functional areas of nursing informatics related to my position

I am a PICU nurse and I float to each PICU/NICU and ER within my hospital. I am expected to know how to complete the charting that is required for each of these units, and they vary depending on which area you are working. This requires me to have knowledge on the use or our EMR. While I know that this question is related to my position as a nurse, but informatics has had a huge impact on me as a mom. My son has a heart defect and in May had a grand mal seizure in my home. He had an abnormal EEG and is now under the care of multiple specialists. The EMR that we use at the organization I work for also has a feature called “My Chart”, this feature allows me to keep in contact with his providers. The questions I as his neurologist can be seen by his pediatrician, so she is aware of my concerns too. So I feel that nursing informatics and technology use has impacted me personally and professionally.

Formal competencies related to my position.

In the ER they chart very differently than the PICU’s because the focus is different, and they are managing care unit they discharge or transfer the patient. This required me to take an additional class on how to chart when caring for a patient in the ER, along with additional hours of orientation. When our organization initially introduced our EMR we were required to participate in additional training hours each month until the program was implemented. I do not learn best by lecturing so this was not entirely helpful and it was not until I started using the program that I fully understood how to use it and the details of charting.

I feel that I am at the point where I can call myself an “experienced nurse” and can pick out what is important and reliable information from websites and other online resources. I am also capable of gathering information and looking at details while informing the physician. A nurse who is competent in informatics will be better equipped to maintain patient safety.

Developing informatics competencies.

There are so many changes that are happening so quickly in healthcare it is important due to changes in laws. The two laws that had a great impact on healthcare are HIPPA and the ACA. Both of these laws serve to protect patients. I would think that to meet requirements of the law, there must be a person in charge of communication between someone who understands the law and the nursing informatics specialist, someone with a certification in the field preferably. It is also essential to know where each nurse stands in relation to how well they know how to use technology and where their confidence level lays. With the aging workforce you may end up with nurses who are not entirely comfortable in working with a computer. There needs to be different levels of education to meet the needs of the new nurse who has learned these skills in their program versus a nurse who is not entirely familiar with technology. It may be a good idea to give these individuals a self-assessment to determine where their strengths and weaknesses lay.
