Obtain both living will and a durable power of attorney documents from your place of work, the Internet, or other source.

  • Complete both documents, including obtaining witness signature. (If a notary public is required by your State law, locate one and record the location and cost of service. You do not need to actually have it notarized for this course.) Texas does not require them to be notarized.
  • Discuss your advance directives with the person you would like to make healthcare decisions for you. If you have no one, discuss with a classmate or coworker.
  • Include a citation to the site where you retrieved your advance directive (if you obtained it online) or from your reading on advance directives.

Where did you obtain your advance directive forms? How accessible were the documents?

I obtained my forms from my unit filing cabinet where we have these forms readily available. One of the questions our unit asks to new admissions is whether they have an advance directive, and if they do not have one whether they would more information on obtaining one. If the patient agrees, we have these forms at the front of our unit at the main nursing station for them to fill out, and a notary that can be called to finalize the document.

If you had to get them from someone else, did they have them readily available? Did they know what you wanted?

When obtaining advance directives in the past, I purchased the forms from Office Depot, and then had them notarized at the bank. When asking an employee where these forms could be found I was led to an aisle labeled “Important Documents”. In the well-marked area of Office Depot, there is also several other forms that can be purchased, including Medical and Financial Power of Attorney.

How difficult do you think it would be for an older adult who is a layman (non nurse) to obtain and complete these forms?

Depending on the level of functionality of these people, I do not think it would be difficult to find these documents. I obtained the forms at Office Depot before I ever considered being a nurse and found them easily. I did not initially know that Office Depot had these forms and was informed by someone at a hospital where I might acquire them. If I did not encounter this knowledgeable person, I would looked to the internet, but for many individuals internet is not accessible so I believe it is mostly up to healthcare professionals to educate others in the pursuit of these documents.

Comment on the benefits of such forms for your family or the healthcare team in determining what you would want if you couldn’t speak for yourself.

Based on the instances where families need to make decisions for patients, it is significantly easier for the families to make decisions for their loved one when there are written forms for them as well as the healthcare team to reference. It is always an emotional decision but when the wishes of their loved one are written in ink, it makes it harder for them to go against their wishes and begins the grieving process with less anguish due to indecision.

What was the reaction of the person with whom you discussed your advance directives? Did the reaction surprise you?

The person with whom I discussed my advance directive was not surprised because we communicated our wishes well before this assignment. It is not a taboo topic amongst our household as we both have experience in helping our family make medical decisions and understand the impact of expressing these wishes in advance.

How have your knowledge and opinion of advance directives changed as a result of this exercise?

This exercise has not changed my knowledge and opinion of advance directives, I have seen how they can affect lives and families both personally and professionally. I will continue to educate my patients, family, and friends about the benefits of advance directive to help ensure that people lead their best life, even up to their death.

I obtained my forms at my place of employment. The hard copy forms/documents were easily accessible since we have them available for any patient that desires to complete one.

I did not have to get them from someone else. These forms are readily available on our unit and facility and I would say most of the staff knows what these forms are and where to find them.

How difficult do you think it would be for an older adult who is a layman (non nurse) to obtain and complete these forms?

I think that the degree of difficulty in obtaining and completing these forms varies from person to person. It would depend on knowing where they can acquire them, who can help them obtain the forms, access or availability of internet to use and print. Some individuals may not know how to read or write and may need help in completing these forms, or may have trouble understanding the information presented in these forms.

Comment on the benefits of such forms for your family or the healthcare team in determining what you would want if you couldn’t speak for yourself.

I think it would be greatly beneficial to have these forms completed and available in the event that I could not speak for myself. There are two different people in my life that would wish to make decisions for my care in the event that i could not do it for myself. Neither one of these people would want to make the choices that I would want carried out. In this case, my advance directives would make it clear and known what my wishes are.

What was the reaction of the person with whom you discussed your advance directives? Did the reaction surprise you?

The person I discussed my advance directives with did not wish to listen at first, but realized this was an important topic to talk about. The reaction did not surprise me from this person, as he usually dislikes talking about death or dying and avoids it at all costs.

How have your knowledge and opinion of advance directives changed as a result of this exercise?

My knowledge or opinion of advance directives has not changed as a result of this exercise. I believe that it is important for everyone to be aware that there are many benefits of completing and having advance directives available, no matter their health condition, age or gender.
